Part Four · Adverse Events, Warnings & Half-Life

Gastrointestinal Tolerability, Biliary Risk, and Retinopathy

What the 2021 safety review and human outcome trials report on transient nausea, biliary disease, diabetic retinopathy complications in SUSTAIN-6, unconfirmed cancer signals, and an elimination half-life of approximately 1 week (~5 weeks in circulation after the last dose).

Nausea Incidence and Cholelithiasis Across 0.5 to 2.4 mg

Clinical safety evaluations of oral and subcutaneous semaglutide center on transient gastrointestinal symptoms, biliary disease, and low-incidence cancer surveillance, as synthesized in the 2021 comprehensive safety review by Smits and Van Raalte in Frontiers in Endocrinology. Across therapeutic doses of 0.5-2.4 mg subcutaneous and oral, the review concludes that semaglutide has an overall favorable risk/benefit profile in type 2 diabetes, with mostly mild-to-moderate transient gastrointestinal effects and nausea in roughly one-third of patients.

Beyond transient gastrointestinal effects, the 2021 safety review reported an increased risk of biliary disease, specifically cholelithiasis. Meanwhile, for pancreatic and thyroid-cancer signals, definitive conclusions cannot yet be drawn owing to low incidence.

Pharmacokinetically, the 2024 StatPearls drug monograph states that both oral and subcutaneous semaglutide have an elimination half-life of approximately 1 week and remain in circulation for approximately 5 weeks after the last dose.

Why SUSTAIN-6 Flagged Diabetic Retinopathy Complications (HR 1.76)

While SUSTAIN-6 in type 2 diabetes (n=3,297) demonstrated a lower rate of major cardiovascular events with once-weekly 0.5 mg or 1.0 mg semaglutide (HR 0.74; 95% CI 0.58-0.95), rates of diabetic-retinopathy complications were higher in the semaglutide group (HR 1.76; 95% CI 1.11-2.78; P=0.02).

In the 2021 Endocrinology safety review, Smits and Van Raalte note that retinopathy monitoring is advised with rapid glycemic correction.

In SUSTAIN-6 (2016), rates of diabetic-retinopathy complications were higher with semaglutide (HR 1.76; 95% CI 1.11-2.78; P = 0.02), and the 2021 safety review by Smits and Van Raalte states that retinopathy monitoring is advised with rapid glycemic correction.
